Nightmare Mode in Dead Rails is the game’s ultimate challenge. You won’t have access to it immediately because it only unlocks after completing a successful run on normal difficulty. Whether solo or with a team, as long as you reach the end and lower the final bridge into Mexico once, Nightmare Mode becomes available to select on the team-size lobby screen.

Alternatively, if you haven’t passed the normal mode before, you can simply join another player who has already done so and unlocked the Nightmare. There are some key differences, so here’s what you need to know about the Nightmare mode:

  • Loot becomes way more scarce. You will find fewer items, which makes money hard to come by and healing tools even more precious.
  • Enemies hit harder, take more damage, and overall become much more dangerous. What might’ve been a simple shootout in normal mode could now become a serious threat if you’re not careful.
  • Passive health regeneration is disabled. That means no more slowly healing over time; you will need to rely entirely on consumables like Bandages, Snake Oil, or class-specific tools like the Vampire Knife. If you’re running as a Zombie, eating corpses is a viable option, too.

While Nightmare Mode strips you of many conveniences, it offers a silver lining: more Bonds. Completing a Nightmare run rewards you with 20 Bonds, double the amount you’d get from normal mode. That’s a solid payout for the risk involved. To make your life a little easier, here are a few survival tips:

  • Stock up before the run starts. Visit the Doctor NPC and grab plenty of Bandages or other healing items.
  • Team up. Running Nightmare Mode with friends or skilled random players makes a huge difference. Focus fire on enemies and support each other during big fights.
  • Be smart about your route. Some areas like Sterling Mines, Fort Constitution, or The Castle become terrible death traps in this mode. If you’re not fully geared or prepared, skip them entirely.
  • Pick the right class. Survivalist is great, thanks to its defensive options. Vampire starts with the Vampire Knife, which lets you heal by attacking enemies — perfect for long hauls without regen. And Cowboy gives you a reliable revolver loadout straight off the bat, which is huge in early combat.
