What Happened To Madlantis?
The former ruler of Madlantis, Queen Michelle, was killed during the course of the game. Raymond, who snatched the throne from her, also ended up dying after the battle with Majima. So was Madlantis completely abandoned or someone else took charge of it?
As it’s an important place for the pirates, it’s unlikely that it was abandoned. The next successor might be Tucker, who has major influence in the pirate community. The ending doesn’t clarify on the fate of Madlantis, leaving much to your speculation.

What Happened To Keith?
Keith is a minor boss who repeatedly challenges Majima to revenge battles. He is quite egotistical and wants to get back at him for the humiliation he faced. For this purpose, he even joined hands with Jack the Collector.
Nothing was shown about his fate in the ending, so it’s unclear what became of him. He may have finally learned his lesson choosing to live his life as a fair pirate, but his whereabouts are currently unknown.

Majima is always up to something despite being much older now. He has a lot to do in life and one of his goals does revolve around trying to do everything he can for Kiryu’s sake. If Kiryu ever gets better, Majima might challenge his rival to battles once again.
Majima is also involved in taking responsibility for finding employment opportunities for Yakuza who were left stranded after the families disbanded. Majima is shown to run a construction firm, and he may just continue that as implied by Nishida during the ending.

What Other Giant Creatures Reside In The Seas?
The seas aren’t explored in their entirety, and in Yakuza games, we often come across strange beasts in Hawaii region. In Like A Dragon Infinite Wealth we battled a giant shark and a squid. A similar squid appears in Like A Dragon: Pirate Yakuza in Hawaii.
That’s not all, but in the ending, we saw a Giant Whale popping out of one of the pools in Madlantis. It makes you wonder what else is in store inside the mysterious depths. We might even face more of these bosses in the near future.

Majima moved to Japan when his adventures as a pirate ended, but does that mean Goromaru was abandoned? It’s unlikely that a glorious ship was left to rot after the events of the game, and what becomes of the large crew that Majima gathered?

Noah’s Future
As a sick child, Noah couldn’t go adventuring much and had to stay on the island for a major part of his life. During his journey with Majima, he got into coughing fits many times. To make their travels worthy, Majima promised to find the Lost Treasure as the last portion of their journey. However, it’s unclear if Noah stopped journeying completely.
Noah’s father Jason seemed to have recovered his valor and love for the seas. So it’s not confirmed if he plans to take Noah on his future travels. His sickness might also get better as he grows up, so seeing his future becoming clarified in upcoming games might be worth seeing.

What Will Become Of Kiryu?
Finding a cure for Kiryu was the main reason for Majima to sail the seas before he lost his memories. Apparently, the Lost Treasure of Esperanza holds an Elixir of Eternal Life, which can increase the lifespan of an individual.
Seeing it as the only hope for Kiryu’s condition, he went out to seek it. However, it seems that this Elixir was missing. But, during the final cutscene, they showed that the Elixir was made of Ambergris, which is a resource found inside a sperm whale. Will this plotline be revisited in the next game, and will the characters try to find it is an unanswered question for now.
Kiryu is suffering from a life-threatening lung cancer and is currently fighting for his life.
