The demon king of the Demon Slayer universe, Muzan Kibutsuji was a force of evil and the first demon to walk the earth. From birth, Muzan suffered a rare and fatal disease. In search for a cure, he was given experimental medicine, but this turned him into a demon.
Ever since then, he has cut dies from humanity and began creating a world of his own and turning humans into demons. He also gained immense strength, regenerative abilities as well as immortality, with the only weakness being exposure to sunlight. He then began to look for ways to be immune to sunlight. But did Muzan have a family by his side through all this?

Very little is conveyed in the series regarding Muzan’s family and his past. It is very likely that he may have killed his family off as soon as he turned into a demon. However, the concept of family was never on Muzan’s mind, as all he cared about was himself and to conquer the weakness of sunlight and saw no value in human life. The series does show Muzan with a “fake” family. Being the master of disguise, Muzan was able to disguise himself as a loving husband and father under the name Tsukihiko, he presents himself as a wealthy man with a wife named Rei and a young daughter.
The main purpose of this disguise was to blend into society by maintaining the appearance of a normal family man so that he could avoid the detection of Demon Slayers. Since he showcases himself as a wealthy man, he is able to maintain his influence with the rich, allowing him to operate with humans without drawing unnecessary attention.

The truth about him feeling love or emotional connection towards this family is questionable, as the wife and daughter were nothing more than just his instruments that he could use to remain undetected. He had no interest in human relationships until they served a purpose in his mission. In the series, Muzan is seen walking his wife and daughter in the city where he meets Tanjiro. Tanjiro is able to see through his disguise and calls him out.
Disturbed by this, Muzan swiftly attacks a passerby without anyone else noticing, causing a commotion as he walks away with his family, leaving Tanjiro to deal with the passerby who was turning into a demon. The wife and daughter are never brought up again in the series, and it is speculated that they also may have faced the same fate as all of Muzan’s victims had. This shows that a family meant nothing to Muzan, and he would have got rid of them without a second thought if he had to.

Since one of the concepts in the Demon Slayer series is familial bonds, such as the Spider Family and Tanjiro’s connection with Nezuko. Muzan is seen as the opposite, with his greed for power, causing him to become paranoid and ultimately leading to his downfall. His need for manipulation and control got him nowhere. The series does reveal a little bit about Muzan’s distant ancestors and mentions that Muzan was also from the same bloodline of the Ubuyashiki Family, and that they have a shared ancestry.
