Summary
- This Baldur’s Gate 3 mod implements a ceremorphosis mechanic, giving you seven in-game days to beat the game.
- Every time you long rest, your stats take a hit and you’re one step closer to becoming a mind flayer.
- There are a few ways to delay ceremorphosis, including the various “cure” attempts you can take in-game.
At the beginning of Baldur’s Gate 3, our companions are quite surprised to find that they are not showing signs of ceremorphosis. The game opens with everyone being given a tadpole, after all, so what should follow is a week of excruciating pain as they’re transformed into mind flayers.
You later find out that there is a reason that you and the rest of the gang are unaffected while others aren’t as lucky. However, if you actually want the threat of ceremorphosis hanging over you, then we have the perfect mod for you.

We’re gonna be talking about this game for a decade and more.
Countdown to Ceremorphosis by Nexus Mods user Cheek is exactly that – a mod that kicks off the timer until your character (and everyone else with a tadpole) is turned into a mind flayer. This gives you seven in-game days to beat Baldur’s Gate 3’s story, and even debuffs you each day to reflect the stages of ceremorphosis. If you don’t defeat the Netherbrain in time, then all of the tadpole’d companions turn into mind flayers, and you get a game over.

Honour mode too easy? Chuck this on top too
Every time you long rest, you lose a point in all of your stats. If any stat hits zero you die, even if you’re not full-mind flayer yet.
The mod doesn’t just add the countdown and daily debuffs, it also changes how attempted cures – like letting Volo poke around in your eyesocket – work. Now, they halt ceremorphosis progressing for a night, letting you get some rest. However, it only works once, so you’ll have to be strategic when it comes to choosing which character gets the treatment.
Even with this in mind, I am baffled as to how anyone could beat the game while using long rests so sparingly. I suppose there are other ways to give yourself the benefit of a long rest, but they can be hard to find, especially early on in the game. All in all, this could be the mother of all challenge runs.
