Others chimed in, with Reddit user art_mech saying, “Yeah, I’ve just stopped doing IH on co-op for now. Such a frustrating experience.” It’s unclear what is causing this issue to occur, but it seems to be somewhat frequent for those attempting to play Diablo 4 with a friend in co-op.
